To be able to backstab you must first obtain the correct skill, which can be found in the ninja skill tree as "Surprise Attack" and costs one point of Ninja skill which you can win by using Ninjutsu items, locks of ninja hair and by leveling your skill stats.

In order to unlock it from the skill tree and purchase it, you must first complete the ninja training mission in the Dojo called "The Way of the Ninja: Rookie". Once completed, you can purchase the skill.

Once the skill is in your possession what you have to do to use it is to carefully approach the enemy by holding the R3 button or using the Catwalk scroll to run silently, and press the triangle button to attack.
