The True Ending in Dynasty Warriors Origins is unlocked after you replay a few crucial missions once you complete Shu's quest. How long the route to the True Ending is actually depends on which faction you're using. Honestly, Shu's route is the shortest and starts at the very, very end of the novel. There are very few fights remaining once this route is unlocked.

Dynasty Warriors Origins: How to unlock Shu True Ending Route?
To start unlocking the True Ending Route in Dynasty Warriors Origins, you will have to complete the Shu story. Now, load up the game once more and select Story Mode from any of the inns. Scroll down to Chapter 5 of Shu and continue scrolling through until you find the Golden Banner. It gives a rather ambiguous hint and will prompt you to see if you wish to start off with the Battle of Changban.
Well in this case the secret tip is not so well, uh secret. In this stage Liu Bei and the commoners are retreating, and it's up to you to get your allies to the escape route safely. But in Dynasty Warriors Origins what you'll really want to do if you're searching for Shu's True Ending Route is focus on defeating Cao Cao himself.
Even if you are a high rank, 50-60+, it is a good idea to play on Historian since you will have to solo Cao Cao's Large Force and a bunch of officers. I went for the Wheel Weapon since it's one of my favorite guard breaks, it has a ton of range and reach, and it was the weapon with the most affinities and statistics.
If you want to jump right into the action, I suggest starting with Zhang Fei as your partner. Defeating Cao Cao's officers and saving the peasants will be enough for the first half of the stage; after that, all that needs to happen for the retreat of Liu Bei is for everyone to gather. Don't fall behind on all the popping stealth attacks; everything is still normal.
Then, three armies will appear at the southern, southwesterly rim of the map. Get down there in a hurry and kill the first one, too. By the time you defeat the first wave of enemies, Cao Cao's Main Force will have already arrived. You can now quit Historian and go kill the forces coming from south of the bridge if you're playing there.
Zhuge Liang has set up enough traps to handle most of the Wei warriors, so the retreating party in Dynasty Warriors Origins is not in a great deal of jeopardy. Get the smaller groups in the north contained, and then horse up to take on Cao Cao's massive army. There will be a whole army of both generic and named police you'll need to take on.
It will be quite handy to have many AOE in this situation. Knowing what you should do right after dealing with several Large Forces, try to eliminate as many troops and officers as possible and disrupt the Grand Tactics. Only your troops will be able to support you in this situation. I would recommend using Fire Arrow and Cavalry Charge from a distance.
Fill up on first aid kits, too. It's decidedly possible you'll be swarmed and put under attack at once by a bunch of troops or officers all at the same time. You will be saved by becoming an expert Dodge mechanic. Master flawless dodges will let you gain a large amount of bravery so you're able to spam more and more skills in one run.
Cao Cao's grand tactics are Flaming Ardor destroy 250 opponents and Focused Fury defeat two enemy officers. Neither of these are complicated, but things will go a lot quicker if Zhang Fei comes along on one of them. He will be able to take out a quick and easy 800–900 guys in one circle by just using his Rage Musou and spamming some free AOE strikes then finish with Musou.
The only objective of this fight is literally cutting your way through Cao Cao's forces and defeating him. Of course, if you can take out his Large Force first, things will go way easier.
Now that he's out of the way, you can finally unlock the Shu True Ending Route in Dynasty Warriors Origins by defeating your old comrade in a duel. When Fate Altered appears on the screen, know that it's triggered.
