How to Get Ingenuity Utility Belt in Path of Exile 2?
King in the Mists can be fought in the Atlas on the Realmgate using the 'An Audience With the King' item. Ingenuity is the dropping when slaying him. The monsters inside create swarms, and you need to beat them to reach the last boss encounter. You can get Ingenuity Utility Belt as the reward if you win.
One does not get the Ingenuity Utility Belt at every successful execution since it is one of the five Uniques that can drop at the death of King in the Mists. And on top of that, it drops after every five battles, so this bodes well. His one-of-a-kind tray table is:

There is an incredibly small chance that this item will pop up as a Ritual favor on Ritual maps, but at costs between 2700 to 3900 Tribute, they are prohibitively expensive and sometimes put players in a tight financial spot. For that, you'll want to defer the item in question by heading to the 'Defer' heading within 'Favours' menu and selecting 'An Audience with the King' in order to obtain it. Because of that, it will be coming back at a discount in a subsequent Ritual. Other pre-requisites permitting, it should return in the first four to five Ritual maps. You might as well defer until such time that you're able to afford it.
In exchange for Ritual points on the Atlas and several rewards, or to make a sale, players can request carries of 1-2 Divines from other players if they have completed 'An Audience with the King' but aren't ready to face the endgame boss.
Does the Orb of Chance give Utility Belt in Path of Exile 2?
It is not possible. You cannot obtain the Ingenuity Utility Belt by killing ordinary monsters or using Orb of Chance spells like Astramentis or Polcirkeln since it does not form part of the regular drop table. Nobody can get their hands on it unless you trade with them; it drops exclusively from the endgame variant of King of the Mists.
