In Little Nightmares 3, the journey of Low and Alone through The Spiral is a constant gauntlet of oversized, twisted horrors. While many encounters demand frantic running, some of the most unnerving moments require absolute silence and tactical teamwork. Chapter 4, The Institute, presents one such challenge in the form of the Dentist’s Office, where players must sneak past a deranged and highly sensitive monster to progress.The primary antagonist in this section—a frantic, blind doctor—is trying desperately to tear down a door on the right side of the room. This doctor is acutely aware of any noise, meaning even walking normally will alert them and trigger an immediate chase. Your goal is to escape through a small, collaborative pet door located on the far left side of the office.However, the pet door itself is stiff and requires both protagonists to push it open, a struggle that inevitably creates noise. The key to surviving this puzzle is a classic Little Nightmares trope: distraction.Here is the step-by-step method to successfully escape the terrifying dentist and continue your journey through The Institute.

Little Nightmares 3: Escape the Dentist Office in Institute

Step 1: Crouch, Creep, and Clear the Path

The dentist's office is littered with hazards that will betray your position. The floor is strewn with empty glass bottles that shatter easily, creating a noise loud enough to draw the blind doctor straight to you.

• Immediate Stealth: As soon as you enter the room, immediately crouch down. Never stand or run in this room unless absolutely necessary.
• Move to the Flank: Slowly walk, while crouched, toward the leftmost corner of the room.
• Avoid Debris: Navigate carefully to avoid touching any of the glass bottles on thefloor. Maintain the widest possible distance from the dentist whilekeeping the bottles in sight. Your movement should be painstaking anddeliberate.

Your immediate objective is the pet door on the left, but you cannot proceed until the doctor is lured as far away as possible.

Step 2: Low's Role—Creating the Diversion

Once you and Alone are positioned safely in the leftmost corner of the room, you are ready to set your distraction. This is where Low’s unique toolbecomes indispensable.

• Aim Far Right: Low must use their signature weapon: the bow and arrow. Aim the bow to target one of the far-off glass bottles located in the rightmost section of the room—the opposite side from your exit.
• Fire: Shoot an arrow at the glass bottle. The arrow will shatter the glass with a loud crash.
• Lure the Doctor: The sound is loud enough to immediately catch the attention of theblind dentist. They will hear the noise and come running to the location of the shattered glass, pulling them as far as possible from yourescape route.

Step 3: Alone and Low—The Coordinated Escape

The moment the dentist begins rushing toward the sound of the broken glassis your window of opportunity. The timing is crucial, as the doctor will soon realize the noise was a trick and will come looking for thesource.

• Initiate the Push: As soon as the dentist turns to investigate the distraction, quickly move to the pet door on the left and immediately start interacting with it to push through.
• The Struggle: Pushing the flap door requires the combined effort of both Low andAlone and will inevitably create a loud creaking sound. This sound willalert the doctor to your true location.
• Escape Window: Since you successfully lured the dentist far to the right, you willgain the precious seconds needed to force the door open. Continue tostruggle and push until the door is open wide enough.

If executed correctly, you will manage to push through the door and escape the dentist's office in Little Nightmares 3 just as the infuriated doctor realizes the ruse and begins closing thedistance. You have successfully used stealth, sound, and teamwork tosurvive another terrifying encounter in The Institute.
