All you need to do is find the Doomsayer NPC and then kill whatever he turns into.
These NPCs can be seen kneeling in the middle of the bloody circle. It's easy to track them down because they won't attack you even if you walk past them. Finally, to get their attention, you need to click on the Doomsayer NPC. This can lead to one of two outcomes:
