The first thing you have to do is to go to the Altus Plateau region. To do this you will have to obtain the two halves of the Dectus medallion. The first can be found in Fort Haight and the other in Fort Faroth. With the two pieces you will be able to take the Great Dectus Elevator from Liurnia, in addition, you will also be able to access the dungeons that connect Liurnia with Altus Plateau.
When you are on the Altus Plateau, you will have to go to the old Altus Tunnel. To do this you will have to head north from any of the paths you used to get to the plateau > go down into the ravine towards Shadow Castle, to find the dungeon entrance on the left.
To open the door you will have to use two Swordstone Keys and when you enter, you will have to move towards the large room where two enemy knights are in the background, near a hut. In this hut will be the Troll's Hammer.
