Chapter 4: Dian Wei's Rescue Mission in the Wan Castle Escape
Dian Wei will note that the enemy's numbers are diminishing and a Fate Altered notice will display on screen if you do all of this in less than eight minutes. Guo Jia's Rescue in the Battle of Mount Bailang (Chapter 5)

Chapter 5: Preventing Huang Gai's Betrayal at Chibi
In order to access the real ending of Cao Cao, players must intervene at the Battle of Chibi to prevent Huang Gai's treachery. In order to accomplish this, they must locate Zhuge Liang and coerce him into leaving the battlefield. His starting point is on the south bank of the river, just west of the main enemy stronghold; they can either wait for him to go to the prayer altar in the bottom left-hand corner of the map or capture him there. After killing Huang Gai, the Fate Altered message will appear on screen for the final time. From there, players can proceed with the battle as usual. Now that the Battle of Chibi is complete (if Cao Cao's fleet doesn't burn), they can move on to the actual ending for Wei without having to wait for the Battle of Huarong Path. Unfortunately, it doesn't stray too far from the default ending—though completionists probably won't care.
When players finish the Wei tale with the genuine ending, they will unlock the "The True Path to Power" Trophy / Achievement.
