The first thing you need to do is to access the content of Queen & the Sea.  Read the initial note and get the fisherman in the polluted sewers, then go to Stilt Village. Which you must destroy for its baby version to reach you. The Leghugger can join you as a support item, and occupy an equipment slot either as the Sinew Slicer or grenades, which you can obtain by unlocking it permanently, or when it comes out in treasure chests or stores.

By pressing the assigned button, summon the Leghugger, which will stay with you for the rest of the level. Press the button again and the little monster will extend spikes from its body and perform a zone-of-effect attack. Otherwise it will automatically attack enemies, biting them and causing bleeding injuries.

When it eats completely, it will evolve to its adult form by obtaining your armored shrimp. In this phase, its bites will cause more damage, covering a wide area with its barbed attack.
