Throne and Liberty: How to Enter the Lamenting Underground
The actual location you'll want to enter for this Codex quest is the Level 40 Cave of Desperation dungeon, despite the text saying Lamenting Underground. Since Lamenting Underground isn't even a dungeon, it's highly likely that this is just a translation error. While dungeons are designed for a group of six people, Cave of Desperation is actually a Level 40 dungeon. Therefore, you can solo this Codex quest if you are at least 50 levels and have enough gear.
To start Cave of Desperation dungeon-which is incorrectly named Lamenting Underground in the mission description-simply open your Co-Op Dungeon screen from the menu and select Cave of Desperation. Simply hit the Enter button at the bottom of your screen to enter the dungeon solo. To do this with a group, go to the Party Matchmaking screen and then select your role, or go to the Party Board, then, under Select Cave of Despair search for a group of people to do it with.
Note that you are unable to complete this codex quest in the Cave of Destruction, even though it is simply the Level 50 version of the same dungeon. This is because it lacks the Paola's Research Journal.

Cave of Despair: Paola's Dimensional Circle Research Journal The quest for the codex can be found right after the first room of the dungeon. To get to the upper level of the Cave of Despair, take the grappling point directly in front of you when entering. Before the wall beside you collapses to give way to the next area, you'll have to fight three waves of ants here. Should you happen to die at the claws of these Ants, do not worry, as the entrance of the dungeon is quite nearby, from which you can resurrect and continue with the battle in a twinkle of an eye.
After passing through the hall, into the next region where the wall falls, there will be a campfire and a Gigantic Moonstone that can be mined. On one's way through, instead of continuing forward to enter the dungeon, take an immediate left into the small room once one's passed the campfire; on the edge of the cave, pulsating with the presence of large orange ant eggs, is Paola's Research Journal. To find it by another means, you can look at the screenshot up top of this post.
You will have found all of Paola's research quests if you found her study journal in the "Lamenting Underground" and completed the Dimensional Circle: Cave of Desperation Codex quest. Rewards upon completion of Chapter 4 of the Adventure Codex are:
