The first thing you have to do is go to Liurnia. Liurnia, north of Limgrave and Stormveil Castle. You can get to this area by going through Stormveil and taking out Godrick. Which will allow you to get close to a Site of Grace.

You can also take the path east of Stormveil and head to the broken bridge that is near the path to the Margit boss fight. From the bridge, you must jump to the cliff and continue along the cliffs until you reach the same area you would exit after finishing Godrick.

Once in Liurnia, you will have to go northwest towards the lake area > continue northwest until you find a small island with albinauric soil.

On the island, you can find the Crystal Tear, which you can use after resting in a Place of Grace. After which you will be able to access the menu to modify the content of your Wonderous Physick Flask.
