In order to find this item you will have to unlock the access to the Eternal City and its main area Nokron. To get to this area you will have to complete some Blaidd and Ranni quests and activate the Grand Lift of Dectus. After finishing with Radahn in Redmane Castle, you will be able to go to Nokron and from there, you will have to advance through a bridge until you reach a four-point junction.

From this point you will have to advance along the path that goes north, which will take you to Hallow Horn Grounds > when you reach this location you will have to head northwest to a broken bridge, from which you will have to jump to the stadium below and climb the wall that is to the east > go ahead until you find some ruins. At this point you can jump with Torrent's double jump ability to stop the bridge and access the ruins.

In the ruins you will have to kill the Ancestral Follower, to be able to access a chest inside which will be the Head of the Ancestral Infant.
