The first thing you need to know if you are wondering how to use the Pureblood Knight's Medal in Elden Ring, is that this item can be obtained by completing one of the White-Faced Varré quests, this quest will allow you to obtain the Pureblood Knight's Medal after completing it.
Some items will allow you to unlock new locations or optional bosses, against which you can fight for additional rewards. With this medal, the manager will let you know that you will have to use it later and nothing else. Varre will tell you that you can use it to talk to Mohg and to unlock a secret location.
When you have it, you will have to activate it from your inventory, so enter active and confirm. This will take you to the unlocked location which is Mohgwyn Palace, where you will have to fight Mohg, Lord of Blood, then explore the new location.
Upon finishing Mohg, Lord of Blood, you will receive a Great Rune as a reward, so the Knight's medal will allow you to access it.
