The Idol From Below is a random Horadric Jewel that you can unlock and then craft. Here's the process:

Acquire Horadric Tomes:

• Seasonal Questline: Your primary way to unlock the initial recipes for Horadric Jewels is by progressing through the Seasonal Quests. You are guaranteed to receive a Horadric Tome as a reward upon completing Tier 10 of the Seasonal Quests.
• Horadric Strongrooms: After receiving your first Horadric Tome, you can acquire more by successfully completing Horadric Strongrooms on Torment I difficulty and higher. These strongrooms are timed micro-dungeons found within Nightmare Dungeons. Each Horadric Tome is a consumable item that, when used, will randomlyunlock a Horadric Jewel recipe, including the Idol From Below.

Craft the Idol From Below:

• Once the Idol From Below recipe is unlocked from a Horadric Tome, you can craft it.
• Crafting Location: Head to the Horadric Lapidary in your town square. This specific crafting vendor is located in Cerrigar, directly to the left of where you claim your seasonal reputation rewards.
• Crafting Cost: To craft one Idol From Below, you will need:
  • 3,000 Horadric Phials: This is the seasonal currency for Season 9, acquired from variousactivities including Nightmare Dungeons, Escalating Nightmare Dungeons,Strongrooms, and Lair Boss Chests.
  • 15,000 gem fragments of any kind: You'll need a substantial amount of gem fragments (e.g., chipped,flawed, regular, flawless, royal) from any type of gem (ruby, sapphire,emerald, etc.).
