Before update 0.3.1 was released, this pickaxe could be found inside a skull tomb in the desert of Azeos. This was kind of tricky, but even with the update coming it's still tricky, so pay attention.
Now to get this pickaxe you will have to face and kill Azeos the Sky Titan, who will drop it upon death. You will be able to meet this enemy at one point in the story as you progress.
You can help yourself with the Azeos the Sky Titan scanner to locate this boss on the map, and you will know that you are in the right place as the arena is fully lit, there are many bird droppings and feathers on the ground and the shadow of a bird giant multiple times.
For Azeos to appear you will have to use a large shiny object, which you will have to place anywhere in the arena, and you will have to wait until its shadow reappears. To make this item, you can go with the Spirit Merchant that appears by placing the Ancient Hologram Pod on the ground.
Taking out Azeos will drop a Titan Chest and an Azeos Soul, but there will only be a 4.12 percent chance that this Ancient Pickaxe will drop.
